چکیده

Discogenic pain is closely related to degenerative disc disease (DDD). The source of low back pain is internal disruption of the lumbar intervertebral disc. Chronic segmental instability, mechanical irritation of sensory nociceptive terminals and biochemically activation of inflamatory mediators are thought as causes of discogenic low back pain. Acute and chronic spinal instability are known as important causes of low back pain. Chronic instability is usually associated with degenerative disease or the long term sequale of trauma, tumor or infection. Chronic instability is accepted as the main problem in lumbar DDD but its clinical and radiological festures are poorly defined. The lumbar instability can be divided into two groups as a clinical (functional) and biomechanical (radiographic) instability. Clinical instability is defined that the degeneration of disc is caused to pain depending on the instability. However there is no radiological findings as angulation or certain degree of listhesis. Degeneration is the disruption of the stable structure of disc which leads to the back pain. The definition of biomechanical instability is used when the disc degeneration, translation and angulation are found on radiologic studies of patients. If there is an intervertebral angulation greater than 15* and a vertebral translation greater than 4 mm on dynamic radiographs, an instability should be thought. 1) Clinical findings and differential diagnosis

Treatment of 44 Cases With Lumbar Spine Stenosis and Degenerative Instability: Outcomes of Surgical Intervention

Background and Aim: Degenerative lumbar spine disease can lead to lumbar spine instability. The patients can present with Low Back Pain (LBP), radicular pain, and motor and sensory dysfunction. Age >50, female sex and pregnancies are among prevalent risk factors. The degeneration process usually starts from the intervertebral discs progressing to involve facet joints, ligaments, and vertebral b...

Perioperative complications in transforaminal lumbar interbody fusion versus anterior-posterior reconstruction for lumbar disc degeneration and instability.

OBJECTIVES Multiple different approaches are used to treat lumbar degenerative disc disease and spinal instability. Both anterior-posterior (AP) reconstructive surgery and transforaminal lumbar interbody fusion (TLIF) provide a circumferential fusion and are considered reasonable surgical options. The purpose of this study was to quantitatively assess clinical parameters such as surgical blood ...

AxiaLIF system: minimally invasive device for presacral lumbar interbody spinal fusion

Lumbar fusion is commonly performed to alleviate chronic low back and leg pain secondary to disc degeneration, spondylolisthesis with or without concomitant lumbar spinal stenosis, or chronic lumbar instability. However, the risk of iatrogenic injury during traditional anterior, posterior, and transforaminal open fusion surgery is significant. The axial lumbar interbody fusion (AxiaLIF) system ...

Comparison the Recurrence Rate of Intervertebral Disc Herniation in Two Surgical Methods of Hemi laminectomy and Partial Laminectomy

Background & Aims: Between 5% and 15% of patients with low back pain suffer from lumbar disc herniation, so intervertebral disc disease is one of the most common causes of low back pain. Surgical intervention for lumbar disc herniation is recommended in patients with severe symptoms and has hopeful results. Although surgery can reduce pain and improve physical function in the short term, its lo...
